Our practice appreciates your trust and confidence in selecting us to assist you in meeting your dental goals. At Dr. Daisy Chemaly & Associates, we offer high quality specialty care at the most reasonable cost to our patients and we do our best to inform you of any financial charges before your appointment.
Below you will find information about how we work with your dental insurance. If you do not see your preferred method on our list, please call our office to find out what other options may be available to you.
Dental Insurance:
We will do everything possible to help you understand and make the most of your dental insurance benefits. As a courtesy, our office will complete and submit your insurance forms to help you achieve the maximum reimbursement to which you are entitled. Please bring your insurance information with you at your initial visit at our practice so that we can expedite reimbursement.
Depending on your insurance carrier’s payment policy and fee guide year upon which they operate, there may be discrepancies in our fees charged and those paid by your policy. We urge you to read your insurance policy so that you are fully aware of coverage and any limitations of the benefits provided. Please remember you are ultimately responsible for all expenses incurred.
Dr Daisy Chemaly and Associates uses the current Oral and Maxillofacial Surgeons fee guide as established by the Ontario Dental Association.
Please note: As a specialist practice, all fees incurred are paid at the time of service.
Dr. Daisy Chemaly & Associates does not accept payment from your insurance company.
Payment options:
Deposits & Payment Installments:
Our office will request a monetary deposit for treatment that involves a third party (laboratory). Since dental prostheses, appliances and surgical guides are customized and made specifically for you, items cannot be returned and payments for such are non-refundable.
For extensive surgical treatment, a deposit will be required prior to the commencement of your treatment. Final payment is due in full, upon treatment phase completion.
