First Visit

Our goal at Dr Daisy Chemaly & Associates, is to provide you with excellent care in a comfortable and relaxed setting. Your general dentist has trusted us with your care and we are committed to delivering and exceeding your expectations.

Consultation visit will involve a thorough diagnosis, explanation of recommended treatment and options, including risks and benefits of treatment. Our goal is to ensure that you receive thorough information and recommendations, tailored to meet your specific needs. Our time investment in patient education enables us to render the best possible outcome for your dental treatment.

Occasionally, treatment can be performed the same day as the consultation. However, this is up to the discretion of your treating provider. A complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment on another day.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medications (i.e., heart medications, aspirin, anticoagulant therapy, bisphosphonates etc).

Before your initial appointment:

Please complete our New Patient Registration & Medical Questionnaire prior to your scheduled appointment.

If you are unable to complete the form in advance, please arrive 30 minutes prior to your scheduled appointment time.

Please provide the following information at the time of your appointment:

  • Referral slip
  • X-rays and/or CBCT scans if applicable (within 1 year). This will assist our doctors in evaluating and preparing for your visit at our practice.
  • Personal and medical information is also required as well as a list of all current medications that you are taking.
  • Insurance information for electronic and/or manual submission.
  • Government Issued Identification (Health card, passport, drivers license, etc.)
    IMPORTANT:     All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.

X-Rays

If your dentist or physician has taken x-rays you may request that they forward them to our office. In most cases x-rays can be emailed to our office at info@drdaisychemaly.com. If additional x-rays are necessary they can be taken at our office.

Parking

We offer free parking at our facility. If the parking is full, there is a parking on Larkin St. Please be careful about the parking instructions on the street.

After consultation, one of our patient care administrators will further explain and review what is involved with your treatment, all associated cost, preparation of an estimate and will schedule your appointment for surgery. You will have an opportunity to address any questions or concerns at this visit.
